HOURS: Monday- Friday 8:30 AM-5:00 PM (On call 24 hours d day)
PURPOSE: Provide palliative (for comfort) care for the terminally ill patients and their families in the final stages of the illness. Focus is quality of life and care is provided in the patient’s home environment or in a local nursing home.
SERVICES: An interdisciplinary team of professionals and volunteers that provides social, spiritual, physical and emotional support. Nurses, social workers, certified nursing assistants, chaplain and volunteer plan and administer care, as well as support and education for the patient’s family to be able to care for their loved one.
ELIGIBILITY: Doctor’s statement that the person has a terminal illness with an anticipated life span of six months or less. Curative measures are no longer of benefit and patient and family have chosen to shift the focus of therapy to palliative care.
FEE/Charges: Hospice provides care to appropriate patients without regard for payment. Reimbursement is provided through Medicare, Medicaid, and many private insurances.
